Photoshop faking advice

I've had photoshop for a while and I know the basics, layers, paths etc. but I've seen some really clever fakes where people have distorted the mouth and added cum to faces etc.

Can anyone educate me as to how to produce a really realistic fake of this kind?

one trick I've heard is that you can use photos of cumshots, then clone the cummy area onto your fake.
